Premature Ejaculation: Causes and Treatments

Premature ejaculation indicates the loss of voluntary control of the ejaculatory reflex.

Some experts define premature ejaculation as ejaculation that occurs less than 2 minutes after penetration or in less than 10 impulses. Delayed ejaculation is another health problem and means a persistent failure of ejaculation, in the presence of a satisfactory erection, is the inhibition of the ejaculatory reflex.

In practice, a man who cannot tolerate a high level of sexual arousal without reaching ejaculation immediately is considered a premature ejaculator, giving him the feeling that he would like to ejaculate later and not at that moment.

The origin of Premature ejaculation

Premature ejaculation is a health problem of multifactorial origin, that is, many things can be related to the presence of this alteration. The history of previous sexual intercourse, the use of medications, disorders such as depression may be related to premature ejaculation. But one of the main factors of premature ejaculation is anxiety. Although part of individuals manage to control it during the sexual act, the vast majority of premature ejaculators are anxious. The problem is that the more repeated these ejaculations are, the more anxious they are, the more adrenaline they produce and the faster they ejaculate. In some cases, anxiety is so much that they end up developing some kind of erectile dysfunction.
No theories about the organic causes of premature ejaculation have been proven.

However, psychological causes also play a very important role and are closely related to the development of this alteration and should always be evaluated and treated. Anxious men who have an episode of premature ejaculation will have a greater tendency to repeat this behavior, as with each sexual intercourse they become more anxious and under greater psychological pressure to perform their role satisfactorily. Many evaluated cases of premature ejaculation show a man’s great concern to provide sexual satisfaction, up to orgasm, for his partner. This pressure will cause more problems.

What are the types of premature ejaculation?

There are 4 types of premature ejaculation:

• Primary premature ejaculation, that is, ejaculatory dysfunction that persists since the first sexual experience;

• Acquired premature ejaculation, when ejaculatory dysfunction begins gradually or suddenly, after a period of normal ejaculatory experiences;

• Natural/Situational Variable Premature Ejaculation, variation from normal sexual performance that may be related to a new partner, an unusual situation or a low frequency of sexual activity;

• Subjective premature ejaculation, subjective perception of rapid ejaculation, but with intravaginal latency time within or even greater than normal, is not considered pathological.

How is premature ejaculation diagnosed?

The diagnosis of ejaculatory dysfunction is based on:

• Conducting a detailed clinical history and physical examination;

• Use of specific diagnostic tools, such as the TLEIV (intravaginal ejaculatory latency time) and questionnaires, such as the PEDT (premature ejaculation diagnostic tool).

Is there treatment for premature ejaculation?

Yes, there is. Premature ejaculation treatment may involve:

• Behavioral measures;

• Psychotherapy;

• Pharmacological therapy.

Treatment should begin with the correction of risk factors, namely, and when applicable:

• Weight loss;

• Practice of physical exercise;

• Treatment of hyperthyroidism and/or other hormonal changes;

• Diabetes control;

• Treatment of prostatitis;

• Abstinence from narcotic substances.

Psychotherapy, accompanied by a urologist or psychiatrist, also contributes to reducing anxiety levels, improving sexual performance and optimizing the couple’s relationship.

Recommendations and suggestions for those with premature ejaculation

» Don’t be shy if you have premature ejaculation and seek the help of a specialist to solve the problem. Sex therapy usually gives good results as well as Authentic Tantric massage.
